Social Worker – Social Work (Full-time/Days) – 0905000106
Description
Demonstrates Service Strategy in all activities. As part of Care Coordination, provides social work services to patients of the Hospital and Creighton Health Professions Center and Boystown National Research Hospital under terms of specific agreements with those organizations. Services include but are not limited to: psycho-social assessments, case management, discharge planning, information and referral services, adjustment counseling, assessment and reporting of abuse or neglect of children or vulnerable adults, crisis intervention, and linkage with community resources. Participates as part of Case Management Team or other multidisciplinary team to assure highest quality services delivered in a timely, efficient manner. Participates in quality improvement activities. Acts as a resource to all hospital personnel regarding social work issues and participates in formal and informal activities. Adheres to NASW Code of Ethics.

Qualifications
BSW required. Master of Social Work preferred. Certified to practice as a social worker in the State of Nebraska. Minimum of one year experience in a health care setting. Thorough knowledge of Medicare, Medicaid, community resources, and applicable State and Federal regulations. High degree of interpersonal and communication skills. Works well as part of a multi-disciplinary team.
