PRN Transfer Center Coordinator Lee County

Job Summary:
JOB SUMMARY: Participates as a member of the Transfer Center in driving communication and coordination of patient transfers throughout the hospital system. The position evaluates patient status / need and determines the appropriateness and priority of the transfer request based on system specialization and hospital bed availability. The TC Coordinator is the first responder for all communication to and from the TC, including doctor’s offices, LMHS hospitals, external facilities, EMS and administrative supervisors. The position is responsible for all necessary communication and preparation between sending and receiving units, and transporters, to ensure a smooth, timely, and coordinated transfer. The TC Coordinator must have a strong clinical knowledge and work closely with internal hospital management, physicians, and EMS dispatchers. Position will also assist the Transfer Center Manager in the further development, tool creation, and growth of TC processes and responsibilities. Individual in this position must be able to handle a high demand, fast paced work environment with a strong ability to effectively multi-task.

JOB REQUIREMENTS:
Education: Registered Nurse
Experience: 3-5 years of health care experience in Patient Care in Critical Care and/or
Emergency Department
Prehospital experience (EMS) desirable.
Case Management, Utilization Review helpful
Training: On the Job
License: Florida Registered Nurse
Other: Preferred ‘ Certification pertaining to job functions
