Yes. It shows you want the job enough that you are willing to go the extra mile and write a cover letter, which shows that you put the effort in!
Make sure the cover letter is specific to the job and comapny you are applying for, and whilst not reciting your CV, using parts of your CV to keep the employer interested in you and make you stand out from the rest of the crowd.
Yes. Call the place and ask if you can fax/e-mail your application instead.
It isn’t a legal requirement or anything, but is pretty standard. I have seen a few resumes come in without cover letters, but the person almost always includes a cover letter. If you are applying online and the system doesn’t give you a place to put in a cover letter, then no it isn’t necessary.
